If you already receive Medicaid, contact your Medicaid case worker to ask whether you may qualify for the Aged and Disabled Waiver. If you do not receive Medicaid, use iServe to apply. We can explain our services and help you understand the next step, but Nebraska DHHS makes all eligibility decisions.
Two separate decisions
If you do not currently receive Medicaid, apply for health coverage through Nebraska's official iServe system.
The HCBS waiver application begins the state's review of age or disability, care needs and Nursing Facility Level of Care.
If authorized for eligible in-home services, your service coordinator helps develop the person-centered plan and provider arrangement.
How to apply
A waiver application is separate from a Medicaid application. Nebraska DHHS reviews eligibility and contacts the applicant about the assessment and next steps.
Start the official iServe applicationOpen Nebraska's official online program-selection page.
Scroll down to the Aged and Disabled section.
Check the box for “Home and Community Based Services (HCBS).”
Answer the questions, review your information and submit the online application.
Nebraska DHHS will review the application and contact you about eligibility, the Level of Care assessment and next steps.
Once approved and when your service plan is developed, tell your Service Coordinator you would like to choose Promising Tomorrows Care Solutions for your authorized services.
